Description
The Sacramento Fire Department is starting a new program called Mobile Integrated Health (MIH), and we're excited to share that the job posting is now live through PlanetBids. MIH is an innovative, community-based initiative and a key component of our alternative response framework. This program is part of our strategy to close critical care gaps and improve outcomes for some of our most vulnerable community members.
The MIH program is a part-time or contract-based opportunity, ideal for advanced providers looking for flexible, meaningful work that directly addresses the opioid crisis, frequent ED utilization, and systemic barriers to care.
We are seeking collaborative clinical partners who are interested in helping to evolve and modernize our service delivery model beyond traditional clinical settings. The program pairs Nurse Practitioners (NP) and Physician Assistants (PA) with a Firefighter Paramedic to deliver high-impact, patient-centered care directly in the community.
In this role, you will deliver high-impact, patient-centered care, managing low-acuity and pain-related complaints, monitoring patients enrolled in managed care plans, providing Medication-Assisted Treatment (MAT), and engaging high-utilizer populations to reduce avoidable emergency department (ED) and EMS utilization. You will work alongside fire-based paramedics and community partners to close critical gaps in care and improve outcomes for vulnerable populations.
Key Responsibilities:
Provide community-based Medication-Assisted Treatment (MAT) services, including initiation and ongoing management of buprenorphine and related therapies.
Identify and manage high-utilizer patients to decrease ED and EMS overuse through proactive care and case management.
Conduct medical evaluations for individuals entering correctional facilities.
Treat low-acuity medical complaints, reducing unnecessary hospital transports.
Support the monitoring and coordination of care for individuals enrolled in managed care health plans.
Utilize electronic health record (EHR) systems for care delivery, consultation, and documentation.
Engage with community partners, correctional facilities, behavioral health providers, and public health organizations.
Participate in continuous quality improvement initiatives and patient outcome tracking.

Requirements
Minimum Qualifications
Education: Bachelor’s degree
Licensure and Certification:
Unrestricted active California NP license with national certification (FNP, AGNP, PMHNP, or relevant specialty), or unrestricted active California PA license with current NCCPA certification.
Valid DEA license.
Current BLS certification (Advanced certifications preferred).
Experience: Minimum of two (2) years of experience, preferably in emergency medicine, field-based clinical environments, addiction medicine, urgent care, or primary care.
Skills:
Strong clinical judgment and independent decision-making.
Excellent interpersonal, communication, and patient engagement abilities.
Technologically Proficient with EHR systems and mobile documentation tools.
Preferred Qualifications
Emergency 911 experience
Family medicine and/or Emergency medicine
Experience providing Medication-Assisted Treatment (MAT) and pain management.
Familiarity with community paramedicine, mobile healthcare, or integrated public safety models.
Demonstrated success in serving high-risk or underserved populations.
Local area contractor
Work Environment
Community-based settings: mobile units, shelters, correctional intake facilities, and residential homes.
(Preferred) Schedule Requirements:
The MIH unit will operate four (4) shifts of ten (10) hours per day. Monday through Thursday, 0800-1800, for a total of 40 hours per week or 160 hours per month. The City desires to hire multiple Nurse Practitioners and/or Physician assistants, where each advanced provider will work a minimum of three (3) to four (4) shifts per month.
This is a part-time or contract-based opportunity. The City intends to hire up to eight (8) advanced providers, depending on program needs. Total NTE for all contracts is $208,000 over the course of a proposed one (1) year contract term. The City may extend each contract to up to four (4) additional one-year terms, for a maximum five-year term.
